The national diabetes program has been in existence for 25 years and currently funds 50 states, District of Columbia, and 8 US jurisdictions. During this time, funding has been gradual, resulting in varying levels of development and capacity among the individual state programs. Additionally, the science around diabetes prevention and control has changed over this time frame and the program has sought to incorporate such changes in its program design. Therefore the program direction has been very dynamic. These and other factors are challenges to efforts to document program effect, particularly the programs contribution to population level changes at the state, local and national level. The Diabetes Indicators Database is a web based resource tool designed to strengthen the capacity of state-based diabetes prevention and control programs (DPCP) in conducting their surveillance/epidemiology/program evaluation activities. The tool will provide DPCPs with resources on essential diabetes indicators and associated national, state and local data sources for each indicator. To promote the optimal usage of this tool, it will be a module within DDT's Management Information System (MIS), with direct access from sections of the MIS that deal with a Diabetes Prevention and Control Programs’ surveillance and program evaluation activities. To ensure data integrity and security, the tool will be password protected on a secure site with access limited to DDT MIS users. | |||
