During this session, we will demonstrate the features of the diabetes database tool, an interactive web-based application to support the surveillance of diabetes care and outcomes. The tool is composed of information about indicators and data sources and allows users to post information about state-specific data sources that they have used to measure diabetes. The tool provides the following general information about each indicator: • A general definition and rationale for using the indicator, • Organizations recommending the indicator (and their websites), • Relevant DDT national objectives, • Relevant Healthy People 2010 objectives, baseline and target measures, • Standards of care related to each indicator (e.g., American Diabetes Association guidelines), and • Links to data summaries and related websites. This information will help users to understand the relevance of each indicator for surveillance of diabetes. In addition, numerator and denominator characteristics for constructing each indicator from approximately three data sources are provided. There are approximately 10 data sources described in the resource tool, including data that provide national as well as state estimates based upon surveys and administrative or claims data. General information about each data source is contained in the resource tool. Specifically, there is information about the population represented by the data source, the purpose and method of data collection, data access and privacy issues, as well as limitations of the data. Further, this interactive resource tool allows users to provide information about state-specific data sources that they have used for diabetes surveillance.

Learning Objectives:

• 1) Use the diabetes indicators tool to identify indicators to monitor the burden of diabetes and the quality of diabetes care
• 2) Use the diabetes indicators tool to find national and state-level data sources to construct indicators of diabetes outcomes and care processes
• 3) Contribute information about state-specific data sources that you have used to measure diabetes in your jurisdiction
