Using Collective Impact Strategies to Support the Bridge Between Public Health and Healthcare

Many states are currently identifying the need for new organizational structures and strategies for linking clinical and population health outcomes. An Institute of Medicine report (Primary Care and Public Health: Exploring Integration to Improve Population Health) emphasized the important role for neutral third party organizations in facilitating successful long-term collaboration across clinical and population health. One solution that is being considered is the Collective Impact strategy. Collective Impact is the commitment of a group of people from diverse sectors around a common agenda for solving a specific social problem, using a structured form of collaboration. NNPHI and its members are creating new coalitions and initiatives that fit within the spectrum of the Collective Impact model. NNPHI and its members commonly function as a “backbone organization” for these initiatives, providing administrative support as well as strategic facilitation, connectivity and resources.
This session will explore new models of Collective Impact within the five conditions (Common Agenda, Shared Measurement, Mutually Reinforcing Activities, Continuous Communication and Backbone Support) related to bridging public health and health care. NNPHI will share the experience of fostering these collaborations among the public health institutes and the session will feature a panel of public health institutes to share their experiences of collaboration in their state to support bridging public health and healthcare through Collective Impact strategies.
Session Objectives: 1) Define the Collective Impact framework, 2) Discuss the practical application of the Collective Impact framework to support shared goals of population health improvement between public health and health care 3) List tools and resources to support Collective Impact endeavors
