Community-Clinic Integration Models in Cuba are widely present and help to support and sustain healthy patients.  Presentation will describe how the Cuban Health system works to systematically connect their patients to a very rich set of community assets and organizations that exist in their communities

Describe Community-clinic integration. Explain how the Cuban health care system is creating formalized links with community partners to address the health of patients beyond the exam room. Discuss the diverse partnerships that exist between health care delivery systems and community organizations and services to promote healthy lifestyles for patients.

I directly observed and met with representatives from across the Cuban Health System and various community groups during my time.
