Social determinants in the healthcare setting: Linking the clinic to the community
With a diverse network of 100+ community organizations, the NeighborhoodHelp™ program empowers households and students to work collaboratively to improve health outcomes and tackle the social determinants of health. This presentation will explore the program and its impact on health outcomes, while discussing how health professionals, community organizations, and academe can work together to implement a social determinants of health approach.

Qualified on the content I am responsible for because: As a health communication advisor at ODPHP, I have worked on highlighting case studies from the field, with a particular emphasis on programs that are addressing the social determinants of health. I have also conducted interviews with members of FIU's Neighborhood HELP program, and produces a video demonstrating their impact on the community.
