Structural characteristics of place are examined to understand their relationship to age-adjusted all mortality in U.S. counties in three time periods: 1968-72, 1983-87 and 1993-97. Of particular interest are the variations between urban and rural locations and among rural counties using nine urban influence categories. Structural variables representing rural composition (ethnicity, income, age) and rural location (population change, medical infrastructure, poverty) are used to predict county level average mortality over five year periods in the 12 state North Central region of the United States.
