Peer Worker Programs for HIV and TB Adherence in Harlem

Harlem is a community beset by poverty, unemployment, and high rates of such diseases as HIV and tuberculosis. Adherence to treatment regimens for these diseases is difficult, due to the number of pills to be taken, the length of treatment, and specific demands associated with the regimens. Drawing on the experiences of various programs in the U.S. and developing countries, we created treatment programs for HIV and for Latent Tuberculosis Infection (LTBI) which utilize peer workers to enhance treatment adherence. The impetus for our peer programs is the belief that patients can be best served when they can relate to a person who has had similar experiences. In our HIV program, the peer workers are individuals who are HIV-infected while the peer workers in our LTBI program have been successfully treated for active TB disease. We are testing these interventions in randomized clinical trials. The panel consists of the Program Manager and a peer worker from each of these two programs. Through the Program Manager presentations, information will be presented on the general design, along with specific issues related to selection, training, and supervision of peer workers. The peer workers will share their experiences in offering services to HIV and LTBI patients, with particular focus on the use of personal experience.
