Connections for Cardiovascular HealthSM Grant Program: Innovations and Best Practices from Community Based Cardiovascular Programs

In 2009, the AstraZeneca HealthCare Foundation (AZHCF), a not-for-profit organization, established the Connections for Cardiovascular HealthSM (CCH) program. Each year, the CCH program provides grants to US-based nonprofit organizations engaged in work at the community level in support of the program’s mission to improve cardiovascular (CV) health. The CCH program reflects a commitment to providing programming at the community level with a focus on connecting people with information, support, and other community members in an effort to produce change in the heart health of participants. This poster discusses various aspects of CCH-funded community- and clinic-based grant programs aimed at improving CV health in communities across the United States. Awardees were able to design innovative programs that were focused on specific target audiences and were eligible for multiple years of funding based on criteria established by the AZHCF Board of Directors. Innovations in community-based health promotion were identified from among the programs that are generalizable and have the potential for replication for other CV health grant programs.
