Expand the Circle of Involvement! The Cook County Department of Public Health's use of MAPP to understand the forces that inequitably distribute the social determinants of health

To fulfill the Cook County (Illinois) Department of Public Health's (CCDPH) mission to “achieve health equity for all people and communities in Cook County,” a team of CCDPH staff carried out a Forces of Change Assessment focusing on the social determinants of health (SDOH). Informed by the National Association of County and City Health Official’s Mobilizing for Action through Planning and Partnerships (MAPP) framework, Healthy People 2020, and the World Health Organization’s SDOH framework, CCDPH conducted four focus groups with a total of 26 people in June and July 2015. Community, labor, and youth organizers, social service administrators, disability rights activists, chamber of commerce, advocates for incarcerated people, renters, food security, immigrants and education were included in the Circle of Involvement.’ The assessment produced themes and information in response to 3 questions about factors, trends, or events affecting the distribution of SDOH among residents of Cook County.

A community organizer will describe his experience in the assessment, the benefits of participating, and recommend actions for building relationships between organizers and health departments. Two health department staff leading the Forces of Change assessment will describe logistics, focus group process, recruitment and characteristics of participants, materials used, data analysis, and lessons learned. This assessment was an important workforce development opportunity for CCDPH staff that created new skills and a context in which to deepen understanding of health equity concepts.
