In recent years, there have been a variety of public policy initiatives focused on community care coordination as an innovative approach to reduce duplication of services while addressing the health and social service needs of those most at risk. The Pathways Community HUB model of care coordination will be discussed.

Discuss the Pathways Community HUB Model of Care Coordination as a risk reduction strategy for achieving the triple aims of better care, better outcomes, at lower costs.
