Certified Older Adult Peer Specialist Program: A peer-based model addressing co-morbidities in older adults living in the community

The U.S. health care system is challenged with providing effective health care for older adults with multiple chronic conditions.  More than half (80%), of adults age 55 years and older have one chronic condition, 50% have at least two conditions, and up to 30% experience co-occurring depression.  The Certified Older Adult Peer Specialist (COAPS) program was created in collaboration with the PA Office of Mental Health & Substance Abuse Services (OMHSAS) with a goal of meeting the complex morbidity challenges of older adults while maintaining independence in the community.  COAPS is a multi-state program that trains older adults in recovery to provide behavioral health recovery services and wellness coaching to older adults in community settings.  Since 2008, over 130 COAPS have been trained representing over 50 counties across the northeastern U.S.  This session will provide an overview of:  1) the COAPS initiative; 2) bi-directional outcomes of COAPS; 3) development of multiple internship partnerships, including an FQHC and HUD-supported senior high-rise apartments; and 4) outcomes of the COAPS program.  COAPS is a collaboration between the PA Department of Aging, OMHSAS and Experience Works, a national training and referral program for individuals 55 and older.
