Building Collaboration to Advance Healthy Aging through a National Prevention Strategy Framework to Improve Health Statewide

A true life course approach to improving population health must include strategies to support healthy aging for the diverse cohorts of adults aged 65 and older, and maintaining a focus on chronic disease and injury prevention, and capitalizing on opportunities afforded by the ACA. This presentation outlines work supporting these goals through the 2014-15 ASTHO Healthy Aging President’s Challenge.

Identify strategies and tactics that state leaders are using to make their state the healthiest in the country and how they are tracking progress. Discuss states’ efforts building public and private sector collaboration to advance healthy aging. Describe policy and systems changes, undertaken for community and clinical linkages, dementia surveillance, addressing caregiver needs and building age-friendly communities.
