In 2008, the Mississippi State Department of Health-Motivated to Live a Better Life: Chronic Disease Self-Management Program (MSDH-CDSMP) was introduced in Mississippi, utilizing grant funds from the National Association Chronic Disease Directors (2008-2011). The goal was to integrate Arthritis Interventions into existing chronic disease and/or other health promotion programs. The Stanford Model of the Chronic Disease Self-Management was the evidence-based intervention chosen to integrate these activities and address all chronic conditions. Since 2009, Motivated to Live a Better Life! CDSMP has conducted over 182 participant workshops reaching close to 1,600 Mississippians. In 2012, MSDH began offering the Stanford model of the diabetes self-management. To become more culturally competent, the MSDH incorporated the Spanish version of Motivated to Live Better Life: CD(D)SMP in 2013, to reach the Hispanic population in the State. Motivated to Live a Better Life sustainability efforts includes partnering with worksite wellness (state agencies, private and public organizations), physician referrals and veterans affairs.
