Reducing Risks Associated with Diabetes: Using a Population Health Driver Diagram

The purpose of this session will be to share the strategy of using population health driver diagrams to address community health challenges. Grounded in the belief that public health and health care are more effective when they combine their efforts to address a health issue than when they work separately, this approach to tackling diabetes engages both sectors and encourages collaboration.

The Public Health Foundation (PHF) and the Robert Wood Johnson Foundation (RWJF) are developing and implementing a Diabetes Population Health Driver Diagram to positively impact this community health challenge increase through collaboration between public health and health care. This project focuses on three local health departments in Texas that are working with PHF to reduce risks associated with diabetes by developing and refining the Diabetes Population Health Driver Diagram. Once the driver diagram is finalized, the health departments will convene community stakeholders to implement activities that address the drivers of reducing risk factors for diabetes.

During this session, the presenters will highlight early lessons learned from the three regions in Texas to depict how the Diabetes Population Health Driver Diagram was used as a framework for determining interventions that can be taken across disciplines to reduce risks associated with diabetes. By the time of the session, PHF and RWJF will have a variety of valuable stories and lessons learned to share with participants to help inform participants’ future ideas about developing and implementing interventions to address diabetes from a community health perspective.

This session will be greatly useful to those interested in addressing the risk factors associated with diabetes. However, because population health driver diagrams can be used to address almost any community health challenge, those looking to increase collaboration and improve their work in other areas of public health will find value in this session.
