For new innovations to be maintained at the community level, not only must they be affordable, but also they need to be culturally appropriate and supported by trained workers and community leaders. This support can have a vital role in breaking the common finding that good project outcomes are not maintained once outside facilitation decreases. Learning organizations within communities that bring together a variety of health workers and cross-sector leaders, such as Community Health Councils and Community Leadership Councils, with reasonable facilitation, can be established, be self-sustaining and have a key role in taking programs to scale, supporting these workers and dealing with many of the current problems that occur when delivering Primary Health Care in developing countries. Ongoing clinical supervision from the local health center is an important part of this process. These points will be illustrated by presentations of 4 successful current projects and programs in a variety of developing countries.
