Strengthening Community Connectivity: Key Strategies to Reduce Health Inequities

The efforts of several demonstration models and the population health approaches used to address social and environmental inequities within select communities will be presented. Sectors within the public health system (i.e. local public health, hospital health system, community health centers and community-based health organizations) will share their unique experiences and perspectives on the critical aspects of establishing partnerships and connections with diverse sectors and community for successful population health.
Session Objectives: Explain the elements of the strategies for implementing model programs to improve the social and environmental conditions that impact health
Describe the unique roles and challenges of integrating the efforts of the public health system in reducing health disparities.
Identify key lessons learned in the development of organizational and cross-sector efforts to address population health.
