Successful control of chronic conditions requires concerted efforts from clinicians, patients, and the health care system. These presentations will describe successes and challenges in community-based chronic disease management and self-care programs; the importance of partnership among the stakeholders; potential economic impact on costs associated with managing chronic conditions; and racial/ethnic disparities in the management of type II diabetes.

Session Objectives: Describe the success and challenges in a diabetes self-management program in the community. Demonstrate the relationship between management of multiple chronic conditions and clinical and community partnerships Identify effective messaging strategies for recruiting persons into a diabetes self-care program. Describe racial and ethnic disparities in glycemic control and their impact on public health among US old adults with diabetes.
