Demonstrating a Framework for Aligning Public Health and Healthcare Efforts to Address Community Health Challenges

Public health and healthcare can work in partnership, with defined roles and responsibilities, to positively impact the health of a community. During this presentation, a population health driver diagram framework developed and tested for successful collaboration and alignment of programs will be presented and discussed. The framework focuses on engaging the healthcare and public health communities to address specific health challenges.  Examples of population health driver diagrams to be discussed address issues such as optimal use of antibiotics, increased use of oral health services, and improving performance of vector control programs. Successes and success factors of this framework also will be discussed.
