The County Health Rankings & Roadmaps program, a collaboration between the Robert Wood Johnson Foundation and the University of Wisconsin Population Health Institute, provides data to measure the health of communities and tools and guidance for implementing solutions that make it easier for people to be healthy. Because health is determined by factors that range from clinical care, housing, education and jobs, it takes leadership from all sectors of the community to create a healthy community. While public health cannot do it alone, public health leaders can play a key role as conveners and strategists to synergize a multi-sector response to the health needs of their communities.

Learning Objectives:
Describe the history and evolution of the County Health Rankings & Roadmaps initiative. List and explain the rationale for selection of indicators used to rank counties’ health status and contributing factors. Identify ways that the County Health Rankings have been used to spur local initiatives to improve health and address social determinants that contribute to communities’ health status.

Qualified on the content I am responsible for because: I am the Co-Director of the County Health Rankings & Roadmaps program at the University of Wisconsin and have led portions of this work since 2009. Prior to joining the team, I worked in local public health for 21 years, working with numerous multi-sector collaborative teams. I hold a PhD in Nursing with an emphasis in public health leadership and have led research studying community health assessment and improvement planning processes
