Designing and planning strategies to address community health needs: Lessons learned from Kaiser Permanente

Describe the strategic processes, tools, and frameworks used by Kaiser Permanente to guide its hospitals in the strategic planning process and identify opportunities for dissemination and adoption of these tools by community and public health stakeholders.
Review and assess the use of the evidence base snapshots developed from the public health literature for use by Kaiser Permanente and partner health care providers in developing evidence-informed Implementation Strategies.
Identify lessons learned and future challenges and opportunities for engagement of hospitals in making improvements to community level population health in collaboration with public health departments.
