Kaiser Permanente's process and tools for examining community health needs
This presentation will provide an overview of how one health system designed and implemented its community health needs assessment (CHNA) process and highlights the new methods, approaches and tools used to guide and support its 35 hospitals across seven regions natoinally. The overview will include a demonstration of CHNA.org/kp, a new web-based GIS data platform created to explore a broad set of health data against the unique service areas of each hospital. The data indicators were selected using the Mobilizing Action Toward Community Health (MATCH) population health framework, allowing hospitals to examine the clinical and non-clinical factors driving the health needs in their communities: social and economic factors, health behaviors, clinical care, physical environment. How community health needs assessments are designed, and their findings, can inadvertently shape the way hospitals think about their communities’ health needs and how to address them. This presentation will review those findings and learnings and provide recommendations for how hospitals can better conduct and engage in a community health assessment processes to advance population health.
