Community based organizations are a critical arm for building momentum towards social change.  If America is to become the healthiest nation in the world by 2030, then communities with shared interests must come together to address the challenges to health posed by social determinants, and do so in a way that assures their own strength and efficiency.   In order for those challenges to be addressed, and for systems change to occur, there must be an interface between health systems and community.  By examining the evidence and experience of how organization, structure, governance, financing, and quality of public health delivery impacts community, there can be a basis for a productive interface with improved outcomes.  For this session, I will discuss major initiatives that were deigned to find interventions that work to reduce disparities in health care, and establish learning networks of providers implementing strategies to serve limited English proficient patients.  I will also discuss my most recent work that is focused on building organizational learning and guiding policy research in a manner that reflects a focus on family economic success, community change and systems reform.
