The Appalachian Diabetes Control and Translation Project (ADCTP) is a joint effort between the Division of Diabetes Translation (DDT) , Centers for Disease Control and Prevention and the Appalachian Regional Commission. The ultimate goal of this project is to reduce the impact of diabetes on high-risk (distressed) communities in the Appalachian Region.

Based on rates of poverty, unemployment, and per capita market income, the counties involved in this project are among the poorest 10% nationwide.  In 2013, 86 counties were designated as “at risk” for severe economic decline. The unadjusted county-level prevalence of diabetes is approximately 13% in these distressed counties (95% CI, 12%, 15%), compared with the national county-level median of 8%.

This presentation will discuss how the ADCTP coalitions, together with Marshall University, have adopted and implemented an evidenced-based model developed by Katie Lorig and delivered by lay community members. Social and environmental changes implemented by coalition members help participants sustain DSME behaviors and improve overall health in these communities
