Background: Long-acting injectable (LAI) formulations of pre-exposure prophylaxis (PrEP) represent the next generation of HIV prevention since they could circumvent the adherence challenges associated with daily Truvada. To strengthen planned efficacy trials, including those with at-risk populations in China, this study measured interest in a LAI formulation requiring an intramuscular injection every twelve weeks among MSM in two cities with annual incidence rates among MSM >5%.

Methods: From September to December 2013, 170 MSM in Beijing (n=99) and Shenyang (n=71) completed a computer-based survey collecting self-reported data on willingness to take LAI-PrEP (the primary variable of interest) as well as sociodemographics and sexual risk.

Results: Three-quarters of the cohort (75.3%) expressed willingness to use LAI-PrEP; those willing were significantly older than those not (mean age 32.4 vs. 29.3, p=.04). Level of education was significantly associated with willingness to use LAI-PrEP (c2=10.3, df=3, p=.02), ranging from 85.0% among those with middle school education or less to 53.3% among those with graduate degrees. At each stratum of sexual risk level, a majority indicated willingness to use LAI-PrEP. Notably, 91.7% of those reporting unprotected receptive anal intercourse (URAI) over the past 3 months vs. 72.6% among those reporting no URAI (c2=4.03, df=1, p=.07) expressed willingness, suggesting that further study of willingness to use and potential promise of LAI-PrEP among those engaging in high risk sexual behavior is warranted.

Conclusion: LAI-PrEP could be a desirable prevention strategy for MSM in China, notably perhaps for those at elevated risk for sexually-transmitted HIV.
