Emotional Distress of Children with Cancer in China: An IRT Analysis of Pediatric PROMIS Anxiety and Depressive Symptoms Short Forms

The Pediatric Patient-Reported Outcomes Measurement Information System (PROMIS) was developed to assess self-reported quality of life and symptoms for children and adolescents experiencing a chronic illness. Eight Pediatric PROMIS short forms have been translated into Chinese and applied in children with cancer in China. This study aimed to describe the psychometric properties of two pediatric PROMIS emotional distress scales -- the Anxiety and Depressive symptoms scales. A total of 232 children and adolescents with cancer were recruited from both inpatient and outpatient clinics in two cities (Shanghai and Chengdu). Categorical confirmatory factor analysis (CCFA) was conducted to assess scale dimensionality and item local independence of the symptom items; and the Multiple Indicator Multiple Cause (MIMIC) model was used to assess Differential Item Functioning (DIF). Item Response Theory (IRT) parameters were calculated from the item thresholds and factor loadings estimated from the CCFA model, and test information against T-scores was estimated for each of the scales. Our results confirmed that the Pediatric PROMIS Anxiety and Depressive symptoms short forms have valid factorial structures; DIF was not present in any item of the two scales for a continuous grouping variable (age) and a dichotomous grouping variable (gender); both the Anxiety and Depression scales have a high test reliability as long as their T-scores are not on the low or high extremes. Our findings provide evidence that the Chinese version of the two Pediatric PROMIS measures can be readily applied to measure emotional distress symptoms in children with cancer in China.
