Biographical Sketch: Jan V. Chacon BS,CHES has managed the Diabetes Prevention Program (DPP) at the Indian Health Center of Santa Clara Valley (IHC) in San Jose, CA since 2007. Jan manages five DPP grants including the Special Diabetes Program for Indians which is a translation of the evidence based diabetes prevention program sponsored by the Indian Health Service. Through Jan’s leadership and vision, IHC's DPP has been the recipient of Six National Awards including the 2011 APHA Archstone Foundation Award for Innovation (hon. mention), 2011 UIHI Leadership in Data use Award, 2010 CDC Frank Vinicor Award of Excellence, 2009 ADA Voices for Change Award for program Innovation, 2009 National Indian Health Board Local Impact Award and the 2008 YMCA Activate America Award. IHC’s DPP was featured in the International Diabetes Federation’s 2010 October issue Volume 55 | Issue 2 of the Diabetes Voice magazine as well as the ADA’s 2009 September issue of the Diabetes Forecast magazine. Jan continues to develop IHC’s DPP and in 2010 piloted a successful DPP cohort funded by a medical care insurance provider for Santa Clara County employees. This constituted a major step forward toward the goal of policy change for prevention reimbursement.
