This session will focus on the concept that health happens where we live, learn, work, and play. Considering these multiple dimensions that create health, health departments and their community partners are working on new ways to address the causes and consequences of disease, form new partnerships to help people live healthier lives, and make policy and environmental improvements that enable people to make healthier choices. This session will focus on partnerships, programs and activities that are ongoing in San Bernardino County and the actions that led to positive change after the 2011 County Health Rankings were released.
