Building Healthy Communities: Exemplars from the Field

The Robert Wood Johnson Foundation’s County Health Rankings & Roadmaps program combines data and action to create solutions that make it easier for people to be healthy in their own communities. The County Health Rankings documents the factors that affect a community’s health, including health behaviors, clinical care, social and economic factors, and the physical environment. The County Health Roadmaps provides guidance and resources to support communities taking action to improve those factors and therefore, ultimately health outcomes in their communities. One component of the Roadmaps is the Roadmaps to Health Prize. In February, 2013, the Robert Wood Johnson Foundation will award the inaugural Roadmaps to Health Prize to six communities throughout the nation. The Prize celebrates the accomplishments of communities who have moved health improvement forward through specific accomplishments with a focus on seven key criteria: leadership; partnership; health equity; focus on the multiple health factors cited in the County Health Rankings model; policy, systems and environmental change; creative use of resources; and evaluation. This session will begin with an exploration of the Prize criteria and their influence on health and continue with the stories of thee of the six winning communities. These stories are intended to provide insights in practical ways to move health improvement forward and inspire participants to return to their own communities invigorated with specific ideas and strategies to continue their journey towards health.
Session Objectives: Describe seven characteristics of a healthy community and how they influence health improvement. Describe specific strategies that Prize winning communities used to improve health in their community. Identify at least one strategy the attendee will take back to their community to further health improvement work.
