This session addresses many issues related to the management of chronic diseases/conditions among the elderly. These include: examining the relationship between hypertension self-care activities and blood pressure in a primary care setting; empowering states to deliver evidence-based disease management and health promotion programs; national study of chronic disease self-management program; and medication (Mis)management.
Session Objectives: 1. To assess patients' self-care activities and then abstracted blood pressure from their medical record. 2. To document how the Chronic Disease Self-Management Program (CDSMP) benefits this population over time.
