This presentation will focus on the challenges of transitions of care on a social, community and health care level. The problems with current outcome measures in transition and challenges that providers and families continue to face in the transition process will be discussed.

Qualified on the content I am responsible for because: I completed fellowships in the Robert Wood Johnson Clinical Scholars Program and the Phillip R. Lee Institute for Health Policy Studies. I am PI for two transition programs with cystic fibrosis and type I diabetes. Along with my survey work, I have performed several qualitative studies of youth, parents and providers on barriers and facilitators to transitioning. I work with numerous community agencies to advance the transition needs for those with developmental and physical disabilities
