Trends in integrated, interprofessional care provide opportunities for maximizing the future of vision in public health

This session will identify trends in integrated, team-based care for people with vision problems, including those at high risk of vision loss. The session will analyze the role of the Patient-Centered Medical Home Model (PCMH) in relation to this population and will evaluate the growing impact of interprofessional education competences in integrated care, using an integrated multi-sensory model as an example. In addition, this session will provide an evaluation of future strategies for greater integration of vision in the context of these trends.
