The Multiple Chronic Conditions Strategic Framework (MCCSF) to improve the health of individuals with multiple chronic conditions, goal 2 emphasizes maximizing self-care management. The National Prevention Strategy's (NPS) goal is to increase the number of Americans who are healthy at every stage of life. Healthy People 2020 (HP2020) goal 2 is to achieve health equity, eliminate disparities and improve the health of all groups; and goal 3 is to create environments that promote good health for all.

The purpose of our study was to evaluate the potential of the Patient Protection and Affordable Care Act (PPACA) combined with the MCCSF, NPS and HP2020 to reduce disparities in care of older Americans by increasing the proportion of older adults with clinical preventive services and those with MCC confident of self-management. Our evaluation emphasizes the potential of this powerful triad (MCCSF, NPS and HP2020) to improve the health of older Americans by reducing the proportion with unmet need for long-term services and supports thru provisions of the PPACA, to enhance the MCCSF, NPS and HP2020 targets of 10% improvement in prevention and reduction of chronic diseases for older Americans. Thereby, having a strong potential to reach longevity among older Americans.

After analyzing PPACA provisions, we conclude that high-risk pools for pre-existing conditions and insurance rescission bans have considerable potential to reduce disparities in access, care and services for older Americans. Overall, the PPACA has potential to reinforce this triad regarding strengthening equity and protecting the precious lives of older Americans.

Describe how the MCCSF would promote quality of life and optimum health for older adults with chronic conditions thru the PPACA; Demonstrate how the combined effort of the PPACA, MCCSF, NPS and HP2020 goals would reduce disparities in care of older Americans by increasing the proportion of older adults who are up to date on a core set of clinical preventive services; Demonstrate how the combined effort of the PPACA, MCCSF, NPS and HP2020 goals would reduce disparities in care of older Americans by increasing the proportion of older adults with one or more chronic health conditions who report confidence in managing their conditions; Demonstrate how the combined effort of the PPACA, MCCSF, NPS and HP2020 goals would reduce disparities in care of older Americans by reducing the proportion of noninstitutionalized older adults with disabilities who have an unmet need for long-term services and supports.
