Despite the knowledge that socio-economic status is an important determinant of population health status, the US public health surveillance system has been slow in employing methodologies that enable area-based tracking of SES variability at the community level. The result has been limited geo-spatial comparisons of the effects of SES variability on community health disparities and a delayed construction of area-based socio-economic measures for testing the relationship of socio-economic positioning to health disparities at the community level.

The objective of this study is to examine the effects of SES variability in the twenty-one zip codes of Nashville, Davidson County, Tennessee, on the variability in diabetes prevalence among the different zip codes. The study will construct an index of socio-economic deprivation for each of the zip codes using two are-based socio-economic measures, the Townsend index and the Socio-economic positioning index. Survey data on diabetes from the Metropolitan Public Health Department and the State Health Department, as well as demographic and socio-economic data from the Bureau of the Census, will provide the database for the study.
