Community Health Workers as trainers for health information exchange

Community Health Workers (CHWs) have been used in many venues to bridge the relationship between health professional and patient. For the past three years CHWs at the Center for Healthy Communities in Dayton Ohio have become expert in the use of HIEx™, a web based system used by the Regional Health Information Organization. In HIEx™ data is captured for all household members and includes demographics, disease information, referrals, scanned documents and it gives CHWs the ability to generate completed SCHIP applications. HIEx™ has been built by CHC and has role based access and a full HIPAA audit trail. As CHWs have used the system they have educated their patients about the system and the advantages for the patient. During the past year CHC has begun using the CHWs as trainers of new subscribers, specifically public health nurses, school nurses and community based physician practices. Training has been done on a one to one basis in a real work setting, so it is applied and instruction tends to be very concrete. HIEx™ is moving toward incorporating the Continuity of Care Record which will include health data including medications, immunizations, allergies, physician orders etc. As these modules become available CHWs will be paired with adopting physicians to help facilitate clinical adoption and to shorten the clinician's learning curve. Post-test only evaluation has been conducted with the trainees and results will be presented at the session.
