The Partnership for the Public's Health (PPH) Initiative was an innovative, capacity building initiative designed to promote a community-based public health approach. Fourteen health departments were partnered with 39 community groups across the state of California to improve the health of local communities. Funded by The California Endowment, the PPH Initiative required that each partnership develop a four-year community-driven, action plan based on goals and strategies intended to result in policy changes and health improvement. Central to PPH were the goals of improving the ways health departments work with communities and empowering communities to make changes to improve health.

This presentation will describe the ways in which the PPH Initiative as a whole and the local partnerships involved were able to make progress toward these goals. The findings include key accomplishments, examples of high impact activities, assessments of partnership progress, and factors related to progress. The information presented highlights the final results and addresses issues of sustainability in community-based efforts. The goal is to provide practical lessons and implications to individuals and organizations interested in funding, implementing and/or evaluating community-based health improvement projects. The presentation also will explore appropriate expectations for community-based health improvement projects.

• Describe activities associated with progress toward a community-based public health model.
• List 3-4 factors associated with high levels of progress.
• Articulate 3-4 lessons learned and how those lessons might be applied to future work in community-based public health.
