Under the aegis of the Florida Department of Health's Comprehensive Assessment, Strategic Success (COMPASS) initiative, public health system partners find resources and support for implementing community-based health planning processes. Recognized as the foundation for promoting healthier Florida communities, community health improvement planning is the process of systematically collecting, analyzing and using information to educate and mobilize communities, identifying priorities, gathering resources, and planning actions to impact community health. COMPASS employs the tenets of the National Association of County and City Health Officials' (NACCHO) Mobilizing for Action through Planning and Partnerships (MAPP) strategic planning model as the framework to guide community-based processes. The COMPASS initiative is unique in its systems-level approach and work toward institutionalization of community-driven health improvement planning as a core process for Florida's 67 county health departments and their public health system partners. A number of tools support local decision-making, priority setting, and health improvement action. These include access to public health statistics via the web-based Community Health Assessment Resource Tool Set (CHARTS), workforce skills development opportunities, planning resources, performance measures for system-wide quality improvement and guidance on policy formulation and evaluation. This presentation will provide background on establishing the infrastructure to support community-driven health improvement planning, share examples of tools and resources developed to support planning, and highlight local successes in implementation of community-based planning processes.
