Medical homes: Spreading innovative pediatric healthcare in Illinois

BACKGROUND: Using an outcomes-based, continuous quality improvement (QI) process, the Illinois Medical Home Project (IMHP) provides primary care practices with training and technical assistance to implement the medical home model. A medical home is defined as primary care that is accessible, continuous, comprehensive, family centered, coordinated, compassionate, and culturally effective. The IMHP educates Illinois providers and families about medical home; encourages family involvement in care coordination and QI processes within primary care practices; supports a system of QI in primary care practices through use of facilitators; provides strategies to improve reimbursement for care coordination and other services; and evaluates the impact of the project in terms of provider and family experiences within primary care practices. The IMHP is funded by a grant from the Maternal and Child Health Bureau 2004 to 2008.

METHODOLOGY: Implementing the “Plan-Do-Study-Act” cycle of practice improvement, the IMHP establishes systems within and beyond demonstration practices to support medical home initiatives utilizing the Center for Medical Home Improvement (CMHI) Tool Kit, National Initiative for Children's Healthcare Quality (NICHQ) resources, QI team facilitators, technical assistance, and funding to support practices.

EVALUATION: Baseline practice data was collected and sites were re-evaluated after 18 months using the CMHI Medical Home Index, Medical Home Family Index, modified Family/Caregiver Survey, and ICAAP Practice Assessment Checklist to measure changes in the delivery of care, access to community services, satisfaction with care received, and changes in provider and family competencies. Findings are presented.
