REACH 2010 Logic Model Brought to Life: Practices that change policies for programs funded to eliminate health disparities

The utilization of the REACH 2010 logic model has assisted its funded community-based organizations with identifying their impact on the development of environmental, organizational, and local-level policies. The logic model is an illustration of logical occurrences of the community to address health disparities in racial and ethnic populations. The REACH 2010 logic model has five stages: 1) capacity building, 2) targeted action, 3) community & system change and change among change agents, 4) widespread risk/protective behavior change, and 5) health disparity reduction.

In various racial/ethnic communities, it has been discovered that community & systems change (altering the environmental context where individuals and groups behave) and change among change agents (changes in knowledge, attitudes, beliefs, or behaviors among influential individuals/groups) has specifically influenced policies. Examples of REACH 2010 community & systems changes and change among change agents include the institution of healthy recipes by churches and restaurants, changes in hospital policies to improve appropriate standards of care and referral among disparate populations, and community collaboration with private developers and city agencies responsible for community economic development.
