Community Based Planning using the ICCC Model: Essential Tools for Evidence Based Chronic Care Planning

The Public Health Research Institute (PHRI),the Eastern Maine Healthcare Systems have been developing in consultation with the WHO, innovative approaches to planning and implementing evidenced based chronic care improvements. These approaches are being used and validated in different settings and countries. Using the Innovative Care for Chronic Conditions (ICCC) Framework (developed by WHO) we have been developing, testing and implementing a chronic care planning system that indentifies community chronic care needs and issues, employs best practice processes to implementing change and evaluates and monitors change with process and outcomes measures for practices and patients. There are several rapid assessment tools used in the system to a) identify and prioritize chronic care conditions in populations, b) assess community based resources for patient self management and their linkages to primary care providers, c) assess that status of community level policies for their contribution to chronic disease; d) assess practice infrastructure and operational readiness for best practice improvements; e) evaluate practices in the management of chronic disease compared to evidence based guidelines; and, f) determine patient barriers to care. To date we have used several tools to develop chronic care improvement plans in two regions of the US—the Blue Hill Peninsula in Maine and the Central Kenai Peninsula in Alaska. An evaluation of the tools using these two projects will be described.
