In this presentation, The New York State Assessment Initiative staff will discuss approaches and tools that (1) guide evidence-based practice; (2) help with gathering information that can be used towards building evidence; and (3) disseminate information regarding the evidence.

Among the approaches and tools that will be described are: the online evidence-based course focusing on disparities in birth outcomes; an in-person evidence-based training implemented statewide; the Community Health Assessment Electronic Reporting System (CHAERS) that gathers critical information from planning documents statewide; a web-based tool to assess usefulness of community health assessments that is being developed, the process for identifying promising practices in community health assessments, and the web-based Clearinghouse. The presentation will discuss the impact of some of these approaches, key resources that needed for implementation, and have suggestions for making these resources applicable nationally.
