As our elderly population increases, preventative measures must be taken to decrease disabilities, reduce institutionalization, and increase quality of life. Addressing risk factors such as multiple medications, cognitive impairment, depression, and difficulty with activities of daily living can delay institutionalization and promote independence.

We identified an at risk/vulnerable population through the use of a screening tool. A statewide geriatric resource team, comprised of nurse care managers, pharmacists, geriatricians and/or other disciplines then performed comprehensive clinical assessments and environmental home safety evaluations. Upon completion of these assessments, they developed medical action plans for frail elders, their caregivers and/or their physicians. We examined these medical plans to evaluate what services or actions were recommended to address the needs of frail and disabled elders. We also conducted follow up calls to assess what recommended medical and community services were implemented. Findings include an increased sense of independence, safety and education in those who have an in home assessment.
