Six organizations in a rural, northwest Illinois county joined together to identify and address the most important local health problems. Following the MAPP (Mobilizing for Action through Planning and Partnerships) model developed by the National Association of County and City Health Officials (NACCHO) and Centers for Disease Control and Prevention (CDC), this collaborative effort used the resources of Health Systems Research of the University of Illinois College of Medicine to facilitate the year-long process and conduct in-depth data analysis.

The six partners included the county health department, two health care delivery systems, community college, school district and local United Way chapter. Relying on three broad assessments to examine and quantify health status, community strengths/forces of change and the ten essential services of the local health system, MAPP proved useful, not only as a strategic and data-driven approach to plan, but also for establishing a community-based infrastructure to implement and evaluate activities designed to address the health problems. By assisting the partnership, Health Systems Research was able to provide analytic expertise while connecting a local health improvement process to the training of medical professionals.
