This session will present: The use of evidence based interventions to improve health and outcomes of COPD patients; the use of policy changes to improve programming; A Turning Point initiative and the manner of how the outcomes from an online survey was used to improve infant health. | |||
Learning Objectives: Participants will learn about: 1)Developing community-based infrastructure for assessment, planning and/or evaluation; 2)Describe one model of successful community-driven policy change that promotes health; 3)Identifying tools and practices useful for engaging grassroots community people; and 4)Developing an electronic system for program planning and evaluation. | |||
