Building Community-based Infrastructure: The President's Initiative to Expand Health Centers
The Presidential Initiative to Expand Health Centers is substantially strengthening the safety net for those most in need by extending the availability of primary health care services to new and existing patients. The Initiative’s goal is to create 1,200 new and expanded health center access points to impact 1,200 communities by the end of fiscal year (FY) 2006 to reach an additional six million people. New health center sites, established by either new or existing grantee organizations, are providing primary care access points for populations not currently being served and penetrating deeper into currently underserved populations.
Learning Objectives: At the conclusion of the session, participants will be able to: 1. Analyze characteristics of Consolidated Health Centers and their affect on the delivery of quality health care to underserved populations in the community setting; 2. Analyze issues and partnerships affecting the delivery of quality community-based primary health care; 3. Describe effective community-based health programs and activities for prevention and disease management that affect patient and families behaviors and lifestyles; and 4. Describe effective programs and activities that are improving the delivery of care in many of our nation's communities.
