STEPS to a Healthier US Initiative: National and Community-Level Strategies to Address Chronic Disease
This session will highlight: the Steps to a HealthierUS Cooperative Agreement Program model, strategies, and objectives; strategies of a Steps-funded city to sustain chronic disease prevention and health promotion efforts through a community consortium; a Steps-funded state example of intervention implementation based on the Steps model; specific examples of a Steps community action plan and activities related to the built environment; and application of national program monitoring and evaluation strategies.
