No Soft Money! - A model sustainable health outreach program

Health Plus is a not-for-profit health care plan serving all areas of New York City. Since 1995, Health Plus has operated a community health program with both outreach and inreach components. This community health program was started with funding from a two-year grant that supported a staff of five outreach workers serving the communitites of Southwest Brooklyn. Through continued corporate support and commitment to health outreach as a way of improving access to culturally-appropriate quality health care the program has grown to include 32 Community Health Workers (CHWs), 2 Supervising CHWs and a Manager. Community health workers in this program are members of the communities they serve and function to bridge gaps between culturally diverse populations throughout New York City and the health care services available. The Health Plus Outreach department exists in a corporate structure that keeps the department free of all fund-raising/ marketing responsibilities thereby providing stable funding and allowing for full focus on community service. CHWs with years of experience in the development of this model program will present this abstract.
