This presentation will describe the UAW process to investigate workplace fatalities and show examples of the reports. UAW fatality reports detail the sequence of the events leading up to the fatal events with strong recommendations based on the hierarchy of controls to prevent further fatalities occurring from these hazards. The reports and investigations are unique in the level of detail to identify the system problems related to the causation of the fatality. The fatality reports use extensive numbers of photos to recreate the conditions before the fatality occurred and leading up the fatal event.
