Reporting of communicable diseases to Acute Communicable Disease Control (ACDC) Program by health care providers is fundamental to the prevention, control, and monitoring of diseases in Los Angeles County. In preparing to better respond and control infectious diseases, ACDC initiated a project to enhance the traditional mandatory passive reporting system. Liaison public health nurses (LPHNs) are partnering with hospital staff to assess hospitals’ reporting systems, improve the disease reporting process, and provide consultation, education, training and other public health services. This presentation will describe how the Public Health Nursing (PHN) Practice Model has been applied in the development and implementation of the Liaison Public Health Nursing Project. The model provides a framework for working with an interdisciplinary public health team to assess strengths and needs, identify goals, and develop a strategic measurable plan for improving the disease reporting process in hospitals. The PHN Practice Model is useful in delineating the LPHN role. LPHNs can follow the sequence of the model, incorporating all major components, to develop a plan and articulate the scope of their work in the prevention, control, and monitoring of diseases in Los Angeles County.
