Evaluation of a Peer-Counseling Model for Breastfeeding

Evaluation of a Peer-Counseling Model for Breastfeeding, an Association of Teachers of Preventive Medicine/Centers for Disease Control (ATPM/CDC) funded project, is a community collaboration between an academic medical center, County Department of Public Health, and the sole delivery hospital in the county, to assess breastfeeding status of new mothers. The model has been in place at the Forsyth County, North Carolina Department of Public Health since 1992 via a program called “Best Beginnings for Forsyth County” (BBFC). Women in the WIC supplemental nutrition program who join BBFC (n= 150/month) receive prenatal breastfeeding classes and postnatal breastfeeding support.

We will report data on 4,000 BBFC participants from January 2000-December 2003 analyzing initiation and duration rates of breastfeeding by: mother’s ethnicity, age, geographic location (rural vs. urban), weight, smoking status, marital status, socioeconomic status, household size, history of high-risk pregnancy, language spoken, breastfeeding history, current issues with breastfeeding, baby’s birth weight and gestational age, and Medicaid status.

We will also present newly pilot-tested methods to assess duration of breastfeeding for 12 months after discharge in breastfeeding mothers (n=20) who were and were not BBFC participants, as a way to gauge efficacy and cost-effectiveness of BBFC. Our future, long-term goal is to provide evidence-based recommendations on how to improve, expand and replicate BBFC.

Participants will: 1. Learn about a successful peer-counseling program, promoting breastfeeding in a low-income population. 2. Recognize factors that may support and/or hinder breastfeeding in this population. 3. Learn how a follow-up program assesses long-term (12 month) breastfeeding duration.
