The proposed project focuses on education of diabetes within the underprivileged population in the Chicago area. Preliminary contact has been made with the Interfaith House. This organization is the only respite center in Chicago dedicated to provide transitional care for homeless and low income adults of multi-ethnic and multi-racial backgrounds. Format will be based on a one-on-one weekly meeting with individual patients monitoring their progress and condition. In addition, a weekly group session/workshop will focus on different aspects of diabetes, such as, general characteristics of the disease, prevention, diagnosis, signs and symptoms, management, course, and prognosis. Special attention will be on diabetic neuropathy, and wound care in the lower extremities. During group workshops, guest lecturers from a variety of background, such as podiatrists, primary care physicians, dietitians, and physical therapists, will be invited to speak with the patients and staff members of the facility. The goal of this project is to establish a working plan for diabetic care at the transitional care center, and to provide education for patients, so that they can monitor their condition and have an avenue to seek for additional aid upon leaving the transitional care provider.
